COVID – 19 GUIDELINES
Guidelines for Appointments 
Our top priority at Circle Drive Dental is the safety of our patients and our team. 
In order to deliver your care in the safest manner, 
here are the steps we ask you to take in preparation for your upcoming appointment:

Please answer “YES” or “No” to the following questions:
  1. In the last 14 days have you had a fever or have you felt hot or feverish?
  2. Have you experienced shortness of breath or had trouble breathing?
  3. Do you have a dry cough?
  4. Any other flu like symptoms in the last 14 days, such as gastrointestinal upset, headache, or fatigue?
  5. Have you recently lost or had a reduction in you sense of smell?
  6. Do you have a sore throat or runny nose?
  7. Have you been in contact with someone who has tested positive for COVID-19?
  8. Have you tested positive for COVID-19?
  9. Have you been tested for COVID-19 and are awaiting results?
  10. Have you traveled within or outside the United States by air, cruise, ship, bus or train within the past 14 days?

If you have answered “YES” to any of these questions, PLEASE STAY HOME and 
call our office (507-289-2055) to discuss the appropriate steps to be taken. 

Prior to Your Appointment:
A member of our staff will call you to confirm your upcoming appointment. 
If you have had any symptoms or exposures to COVID-19, 
please make our staff aware to determine if we can keep your appointment as is or if we should reschedule to a future date. 
Upon Arrival:
Masks are required per the MN Board of Dentistry outside of the treatment rooms. 
If you do not have access to a mask, we will provide you with one.
At the End of Your Appointment:
Please call us if you have tested positive for COVID-19 within 7-10 days after your dental appointment so that our staff can take recommended steps to prevent the further spread of COVID-19.
